Please visit the website Application to Work in Canada: Work Permits for applications and information. You can also download the Document Checklist which will give you all the documents needed.
However, I suggest that you send copies of your study permit, a letter from the university indicating your program and its period, and documents to prove that she is your partner (such as marriage certificate) in addition to the other docs. Also you will have to pay money using a special form at the bank and send the receipt as proof of payment with your application.
